The South Milwaukee/St. Francis Health Department is offering rapid antigen COVID-19 testing, by appointment only, Monday-Thursday, 9:00 AM - 3:00 PM. The testing is only available for those who are symptomatic or have been exposed. We can no longer test individuals for travel purposes. The results will be available 15 minutes after completion.
Important update: February will be our last month of COVID-19 Testing. The testing site will close permanently at the end of the day on February 28th.
Please follow all 4 steps below to sign up for an appointment and register for COVID Connect to receive your results.
2.) Complete the screening questionnaire on COVID Connect. This website will provide you with your test results: COVID Connect 2
3.) If you attached an email address to your registration on COVID Connect, you will receive a QR code via email. Please have your barcode ready to be scanned when you arrive.
4.) Arrive to our testing site, 2424 15th Ave, at your appointment time. Please enter through the south-end parking lot (labeled POLICE & HEALTH) and park next to the white sign labeled "COVID testing". Please call the number listed on the sign to confirm your registration. Please stay in your vehicle, a staff member will come out to test you.
